The Propaganda Head is a standalone armor piece that can be obtained in The Nightmare. It can be purchased from Rising Waters at a price of 20,000,000 gold, or from MANAGER SUPREME at a cheaper price of 18,000,000 gold. Other viable methods of acquiring it include the following:
- A possible reward from the Guttermouth Cells at a 0.03% chance.
- A possible reward from The Lost at a 0.09% chance.
In appearance, the Propaganda Head is a ridiculously large cube worn over the head, with colors ranging from red, dark red, white, and black as seen from bottom to top. Each side of the head features incomprehensible symbols and illustrations.
When equipped, the Propaganda Head will provide 0 armor points, however, it will provide a 17% increase to the player's endurance, the highest out of any equipment worn in the head slot, reducing the stamina spent from weapon, tool, or movement usage.

- The Propaganda Head has and can be used to obtain the highest amount of endurance in the game, the highest being 49.50% with Forgotten Life Platebody and Platelegs, Otherworld Boots and Tough Green Ring.
