It is noted that it is also found in the Otherworld Tower, a 50-floor dungeon where a maximum of 4 players can venture through and slay the enemies that lurk in the dungeon. This particular mob can be found on the 46th floor, They Live in the Same House, where a wave of Otherworldly Dollhunters, an "Otherworldly Jester", and the "Otherworldly Doll" will attempt to attack the player(s).

If the player is in the Otherworld Tower, however, they should attempt to stand on the head of the doll-like monster. Standing on the jester-like mob can result in death. Similar to the Ancient Ogre, Yellowman, and/or the Pirates, the Otherworldly Dollhunter are notably seen sharing similar attacks to the other mobs that were mentioned. The Otherworldly Dollhunter is expected to be tougher than other 'normal' mobs that can be found in the Frontier and is deemed as a 'nuisance' or an 'annoyance' due to how it can easily kill the players.

Trivia
In appearance, the Otherworldly Dollhunter possesses similar 'skeletal' structures of a humanoid in the Frontier, except that its 'bone' structure is comprised of metal-like materials from the Otherworld, as well as void-like energies infused into some parts of its body It is also noted that it has four eyes and each eye may or may not glow. It also wields a weapon that is sometimes compared to a rapier to allow the creature to attack the player. Its name originates from how it is from the Otherworld, as well as dropping doll-like eyes and notably known for hunting down unfortunate players.
