The Baltimore Worm is one of the many fishes that can be acquired in the Frontier. It is an aquatic worm that can be found in two bodies of waters known as Blackrock Pond and Black Tower Well respectively.
In appearance, it resembles a long worm creature with its long length. The Baltimore Worm has multiple different colors in different hues and tints. The Baltimore Worm seems to have odd markings on it for rules of a game.
The Baltimore Worm can be consumed by the players, restoring 100 HP back to the players' health pools. The players also have the option to sell it to a selling vendor at a selling price of 25,000 gold.

- It possibly relies on their sense of smell in order to move around.
- The Baltimore Worm is apparently forced to live where it should not be at and is deemed unnatural, meaning that the Baltimore Worm came from someplace entirely.
